Membership Terms & Conditions

Please fill out the appropriate Terms & Conditions for the membership you purchased.  The form will need to be filled out for each member of your membership.  Minors 17 and younger must get a parent or guardian to fill out and agree to the terms.

Month-to-Month

With a Month-to-Month Membership, you’re billed monthly with the card you provide on file.  No contract, no commitment!

With a Pre-Paid Membership, you pay up front for a 1 month, 3 month, or 1 year membership.  No enrollment fee!
